Knee Osteoarthritis in Older Adults: Better Mobility Starts With the Right Plan

Knee osteoarthritis can make stairs, walks, errands, travel, sleep, and time with family harder than they should be. OrthoFairfax provides focused orthopedic and sports medicine evaluations to identify the source of knee symptoms and build a plan around mobility, comfort, and independence.

Recognizing knee osteoarthritis

Knee osteoarthritis is a common cause of pain and stiffness in older adults. It involves changes in joint cartilage and other structures and can affect each person differently. Common symptoms include pain with walking or stairs, stiffness after sitting, swelling, reduced bending, tenderness, and difficulty rising from a chair. Some people reduce their activities because they worry the knee will hurt or give way.

The impact goes beyond knee pain

When walking becomes painful, people may stop taking daily walks, avoid stairs, depend on family for errands, or give up hobbies and social activities. Poor sleep and less movement can affect energy, confidence, strength, and independence. A meaningful treatment goal is not simply to label arthritis on an X-ray—it is to help each person move more comfortably and safely and keep doing the activities that matter to them.

How OrthoFairfax evaluates knee pain

We begin with your symptoms, medical history, activity goals, physical examination, and prior treatment. Musculoskeletal (MSK) ultrasound can provide real-time views of selected soft-tissue structures around the knee, including tendons, bursae, fluid, and other accessible anatomy. It can complement the examination and help answer focused clinical questions. X-rays or other imaging may still be needed to assess joint changes or investigate a different diagnosis. No single test replaces a complete evaluation.

Build the foundation before choosing an injection

Strengthening the muscles around the knee, improving mobility, adapting aggravating activities, and maintaining appropriate physical activity are central parts of osteoarthritis care. Physical therapy, supportive devices, topical medication, and selected oral medicines may be useful depending on a person's health and medication risks. The best plan accounts for balance, other medical conditions, current medicines, and personal goals.

Gel injections for knee arthritis

Gel injections, also called hyaluronic acid or viscosupplementation, place a hyaluronic-acid product into the knee joint. Some patients report symptom relief, but research results are mixed and major clinical guidelines do not all recommend routine use. The potential benefit, product, number of injections, insurance coverage, and out-of-pocket cost should be discussed before proceeding. Gel injections do not restore lost cartilage or cure osteoarthritis.

The power of PRP for selected patients

Platelet-rich plasma (PRP) is prepared from a patient's own blood and used in selected musculoskeletal treatment plans. For knee osteoarthritis, some studies report improvement in pain or function, but the evidence remains variable, preparation protocols differ, and recommendations are not uniform. PRP is not a proven way to regrow cartilage or reverse arthritis. OrthoFairfax can discuss the available evidence, limitations, costs, and whether PRP fits the patient's diagnosis and goals.

MSK ultrasound also has an important role in evaluating selected knee structures and can help guide certain injections when appropriate. Image guidance supports accurate visualization of anatomy; it does not guarantee a particular clinical result.

What about steroid injections?

Corticosteroid injections can provide short-term relief for some patients, particularly during a painful inflammatory flare. They do not cure arthritis, and repeat injections deserve an individualized discussion of potential risks. Steroid, gel, and PRP injections have different mechanisms, evidence, costs, and intended roles; they are not interchangeable.

Choosing the right treatment for your life

Ask: What is causing my symptoms? What improvement is realistic? Which treatments have the best evidence for my situation? What are the risks, costs, and alternatives? How will we measure progress? A thoughtful plan can combine rehabilitation, activity changes, medication when appropriate, and selected procedures. Surgery or another specialist opinion may be worth discussing when symptoms remain severe despite appropriate non-surgical care.

OrthoFairfax is an orthopedic and sports medicine practice in Fairfax, VA, focused on careful evaluation, MSK ultrasound when useful, and individualized non-surgical options. Our goal is to help patients make informed choices—not to offer the same injection to everyone.
